How does this even happen? I've never torqued my bolts and they have never come loose. How hard is it to just tighten the bolts and check them every new tire or every now and then?

If for some unknown reason the bolts aren't done up tight enough when changing the sprocket and then come loose you might as well through the sprocket because the countersunk holes become mis shaped and no amount or torqueing works after that has happened.
I use to always change the bolts at the same time as the sprocket, but had to be careful which ones I got because talon hubs only use to work with nuts without washers .

Don't care if you torque wrench them. Its does NO good unless you use RED Loctite! Takes small propane torch to melt that stuff when replacing sprocket

You need a new torque wrench...the one you used was evidently broken! Tongue

that sprocket is clearly worn-out. I suspect poor MAINTENANCE (!)
how's the rest of the bike?

I am laughing my ass off, while simultaneously cringing about the time my axle broke because of a loose chain adjuster bolt. Blush

Weird mechanicals we have had-

Bought a cheap 01 CR250 and during first ride to kind test it out at my house the front sprocket bolt came out. Sprocket cam off in air and locked up rear wheel. Luckily just skidded out. I checked bike over beofre riding but didn't think to check that.

Brand new 2016 KTM 65 - Inner flywheel lining came loose and bike died in air over 70' table. Luckily son just rode it out. A few seconds later he would've been headed up the face of a 90' step-up. Same bike rear brake rotor exploded while practicing starts. Took caliper and bracket with it. Both around 10 hours on a well maintained bike. Thanks KTM!........

Clean threads - blue loktite - no torque wrench just tighten them as much as you can. I mark them but I have never had an issue - on any bike I've ever owned.

I don't know WTF your guys problems are with sprocket bolts but it's been pretty comical reading you guys discuss it. My question is how does seat bouncing effect sprocket bolt torque?? Peeley??
