Front alignment issue - RM 250

I can't figure out why my alignment is so off. Initially thought it was a tweaked handlebar, but after taking those off its pretty clear by looking down at the clamps/wheel something is off. Could it be axle? Wheel spacers? Are my forks bent?
I will also add that I double checked that the front wheel was installed correctly by compressing the forks.

Probably just twisted is the clamps. Happens often even with small slow tip overs. Usually just loosen up the clamps, bang front tire on a tree or truck tire to get it straitened back up.
